REQUIRED AGE AND GRADE FOR CLASSES:
- PK-JK students are 3 and 4 years old. SK-1 students are 5 and 6 years old.
- Students should remain in the same age group level all year -- they should not switch to a new class level in the middle of the year with a new birthday excepting 2.5-3 yrs who wish to switch.
- The Christ Methodist School is open to the public. Other satellites require that you are a student at their school, except during summer months.

You do NOT have to be a member of the Racquet Club and there are no additional fees or obligations.
Review the available classes and write down the classes and class codes (when applicable) and the associated fees. Next review the information below regarding discounts and waivers.
Then proceed to the REGISTRATION FORM to register and pay.
Register & pay online to ensure quick spot placement. Online registration is only available with CREDIT CARD.
Severability and Venue: The undersigned further expressly agrees that the foregoing waiver and assumption of risks agreement is intended to be as broad and inclusive as is permitted by the law of Tennessee and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. Likewise, I agree that if legal action is brought, it must be brought in Memphis, Shelby County, Tennessee.

If you have a sibling in the CBT program at Racquet Club or Christ Methodist, take $10 off the total price.
Full members of the Racquet Club may deduct $60 from the total of their Racquet Club CBT classes.
ALSO: If you are taking 3 or more classes a week, you have option to pay ½ tuition now

WAIVER OF LIABILITY, INDEMNITY AGREEMENT, AND ASSUMPTION OF RISK:
In consideration of permission to use, today and on all future dates the property, facilities, and services of Children’s Ballet Theater, Memphis Youth Musical Theater, Racquet Club, Christ Methodist School or Church, Roulhacs, St Agnes, Second Baptist or Lipman School Ballet, (CBT programs) I, on behalf of myself, my heirs, personal representatives, or assigns, do hereby release, waive, discharge, and covenant not to sue, CBT programs, its directors, officers, contract workers, volunteers, independent contractors, and agents from liability from any and all claims arising from the ordinary negligence of any agent of Children’s Ballet Theater or any of the aforementioned parties.
I hereby give permission to Children’s Ballet Theater and Memphis Youth Musical Theater to photograph my child and use the photos solely for their own marketing purposes, including brochures, advertisements, ballet website and news articles.
IMPORTANT - REFUND POLICY:
75% refund is given if a student withdraws within 24 hours prior to a second class. No refunds will be given after this time except in the case of a physician -‐ verified illness. There is no pro -‐ rating and no makeup cla sses. Tuition is not transferable to another session. Students cannot attend classes until full payment is made. There is a $30 fee for cancelled checks. CBT reserves the right to cance l classes due to inclement weath er conditions or teacher illness. In th e event of a weather condition, call CBT office one hour prior to your class time at 921 -‐ 0388 for a recorded message. There are no makeup classes for weather conditions.
This agreement applies to 1) personal injury (including death) from accidents or illnesses arising from the participation in our Ballet School activities including, but not limited to, organized activities; classes; observation; performances at GPAC theater, Buckman Theater or any other theater; and individual use of facilities, premises, or equipment; and to 2) any and all claims resulting from the damage to, loss of, or theft of property.
Indemnification and Hold Harmless: I agree to HOLD HARMLESS AND INDEMNIFY Children’s Ballet Theater from all claims resulting from negligence and to reimburse them for any expenses in investigating and defending a claim or suit if my claim is withdrawn, or to the extent a court or arbitration determines that Children’s Ballet Theater is not responsible for the injury or loss.
Acknowledgement of Understanding: I have read this waiver of liability and indemnification agreement and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by the State of Tennessee. The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward’s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth in Section 1. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to in Section 1 from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian. I hereby waive any claim of liability on behalf of Children’s Ballet Theater programs, for any efforts to obtain treatment from a doctor or hospital for my child in event that my representative or I cannot be reached.
